Health Evaluations

Initial Comprehensive Health Evaluation
This is the most important part of homeopathic care. The initial evaluation takes approximately 2 hours to gather the individual characteristics of the complaints as well as the person, such as appetite, cravings and aversions, perspiration, bowel habits, temperature preferences, hormonal functions, sleep, dreams, and most of all, emotional temperament. The past history of the person and their family members is also carefully analyzed to make the assessment complete. A comprehensive exam and a look at any medications and/or supplements you are taking is also performed during the first visit (Note: Dr. Ronda does not do laboratory work). Recommendations will be made at the first visit, looking at supplements, homeopathy, lifestyle and diet. Any recommended homeopathic remedy is included in the cost of the initial visit (up to two individual homeopathics--does not include kits or boxes of remedies). Dr. Ronda carries very few supplements, but she will discuss where they may be able to be purchased on your own (usually a local health food store).
As the first visit is 2 hours long and there is a lot do accomplish in this first visit, filling out the Comprehensive Assessment Form (below) will make the best use of your time during the visit. If you choose not to fill out the Comprehensive Form ahead of time, it will be filled out during your evaluation. If you go over your 2 hour time slot, there will be an additional fee of $20 per 15 minutes charged.
Forms
The forms noted here are in Microsoft Word format and have been set up so that you can tab between spaces or use your mouse to check boxes. Forms must be returned PRIOR to your appointment time or you will not be evaluated when your appointment time comes; you will, however, be charged for the time slot. It is YOUR responsibility to make sure the forms are received prior to your appointment.
Emailing the forms back: You can download the form onto your computer, fill it in with the computer, then attach it to an email and return it to Dr. Ronda via the email: info@DoctorRonda.com.
Sending the form via the United States Postal System: You can also return the form via standard mail. However, if you have an appointment in a few days, you cannot return the form via the mail.
Sending the form via fax: At this time, the fax number and phone number are the same so you will need to call first to alert us that you will be sending a fax, then you can send the fax. The number to call (and fax) is 920-321-0008.
Only the short form is required for the initial evaluation. If you choose not to fill out the Comprehensive Assessment Form, you can fill out the Short Form But know that your assessment time will probably go over the 2 hours.

NOTE: due to an increase in postal fees as well as the amount of our resources that will be utilized to print and send this form via the US Mail, if a form has to be sent to you, a $5.00 charge will be incurred per set; this fee will need to be paid via PayPal or credit card prior to our sending you this form. If you do not have email or access to a computer, most libraries have both internet and printing services available.
Follow-Up Health Evaluations
The follow-up assessments take approximately 45 minutes and are usually scheduled 4-6 weeks apart, with the length between visits lengthening as your health progresses. These visits are very important for determining the progress of the person as well as for determining if the person’s care requires any change of action. During these consultations, Dr. Ronda evaluates, assesses and monitors the person’s health as well as their complaints while taking the homeopathic remedy or supplements. Depending on this assessment, Dr. Ronda makes further decisions regarding dosage, frequency or the change in the homeopathic remedy (up to two remedies per visit). Dr. Ronda ensures that the person is progressing towards health according the Law of Cure by Hering. Each of the complaints is monitored for their frequency, intensity and duration along with all the changes in the person’s health as well as situation in life. The frequency of these evaluations decreases as the person progresses on the path of recovery and health.
Focused Evaluations between Scheduled Visits
For established clients, a focused evaluation is when Dr. Ronda answers questions via phone or email for things outside what has been covered at an evaluation. Examples are: asking about a supplement or procedure you read about; or discussions of your most recent medical doctor visit. These evaluations are charged but at a reduced fee than a standard follow-up visit.
If you have not had a follow-up visit and one was suggested to you, however, you cannot have a focused evaluation.
Acute Concern Evaluation
This class of health evaluations is reserved for established clients only. Acute Concerns are those health concerns that have been affecting you for less than 6 months.

NOTE: In the interest of protecting her clients and keeping the integrity of natural health, Dr. Ronda WILL NOT work with any multi-level marketing company. If you are a representative from one of these companies, DO NOT call, email or send information, as Dr. Ronda will not respond.

New Payment Policy
Appointments will no longer be scheduled without securing the visit with a credit card or a paper form of payment such as a check or money order. We accept major credit cards. There is a 24-hour cancellation/change/no-show policy; you will be charged for the appointment if the appointment is not kept or if it is changed with less than a 24-hour notice. Cancellations or appointment changes via Email will NOT be accepted!

NOTE: Dr. Ronda does not answer the telephone when she is in session with a client; always leave a message and a return phone number (stated clearly as Dr. Ronda doesn't have caller ID so she will not know your phone number if it isn't provided clearly, hence no return phone call will be made).
Dr. Ronda will return telephone calls that are made during office hours the same day as the call; those made after hours will be returned the next business day. Email messages will be answered twice daily ONLY: in the morning at 9 a.m. and in the evening at 5 p.m.
